  • If you are currently enrolled in a FSA benefit and you have not previously accessed your online account, please follow these instructions: Click on the GIC Participant Login button below and on the login screen you can: 1. Create a login with your username as your first initial followed by your entire last name and the last four digits of your SSN (e.g. Joe Smith with a Social Security Number of 123-45-6789 would have the username: jsmith6789). Your password, by default, is "changeme". As soon as you login to the site, you will be prompted to change your password and create security questions. Or 2. You can create your own user name and password by simply clicking on New User - Create your new username and password link.

  • If you have already created your online account, and you have an existing username and password, please use that login information to re-enroll online for 2012 HCSA/DCAP benefits during the Open Enrollment period from October 3 - November 18, 2011.

  • If you are a new hire or first-time enrollee into the HCSA/DCAP benefit, you must complete, sign and date the HCSA/DCAP paper enrollment form (see below) and give a copy to your Payroll Coordinator.

  • Open Enrollment for HCSA/DCAP benefits for the 2012 plan year runs from October 3 to November 18, 2011. Upon completing your online enrollment, please print two copies of your online confirmation page for the 2012 enrollment. Keep one copy for your records. You must submit a signed and dated copy to your Payroll Coordinator by November 18th, 2011. **

    **Important: If you do not submit a signed and dated copy of your online enrollment confirmation page to your Payroll Coordinator, your account and HCSA debit card may be placed on hold if there is no payroll deduction for 2012.
  • HCSA/DCAP Participants: The confirmation email of December 17 only related to Commuter Choice Participants. Your 2012 HCSA and/or DCAP election amount(s) will be online January 1, 2012.

  • HCSA participants enrolled in 2011 have until March15, 2012 to incur expenses to spend down their remaining 2011 balance. If the debit card is used for eligible expenses incurred from January 1 to March 15, 2012, the transaction will debit the 2011 remaining balance first. Participants can also submit a claim form by April 15, 2012 for expenses with dates of service from January 1 to March15, 2012 to be applied to the 2011 balance. All claims must be submitted by April 15, 2012 for the 2011 plan year.

  • DCAP participants enrolled in 2011 have until March 15, 2012 to incur expenses to spend down their remaining 2011 balance. All claims must be submitted by April 15, 2012 for the 2011 plan year.
