• To access your account on line, please follow the instructions below: Your user name is your first initial followed by your entire last name and the last four digits of your SSN (example: Joe Smith with a Social Security Number of 123-45-6789 would be user name jsmith6789). Your password by default to get in is changeme. The default PIN is all lower case, all one word, changeme. As soon as you login to the site, you will be prompted to change your password and create security questions. If you cannot remember your PIN, please click on the “forgot password” option and one will be sent to your email address.
  
  
• Participants enrolled in 2009 benefit with an available balance have until 3-15-2010 to spend the funds or submit a paper claim by 4-15-2010 (the forms must be post marked no later than 4-15-2010 if mailed).
  
  
• If a participant has a debit card from 2009 with an available balance, when using the card in 2010, the transactions will pull from the 2009 balance first.
  
  
• All customer service inquiries regarding claims history or service questions after October 23, 2009 should be directed to Benefit Strategies by dialing 1-877-FLEXGIC (1-877-353-9442) or email us at flexgic@benstrat.com.

ACTIVE/ENROLLED PARTICIPANTS:

Once you log in go to the Profile Tab and click on Profile Summary.  This tab will allow you to change or enter:

• your address
• your email address
• your direct deposit information
• your date of birth
• your gender
• your marital status
• your dependents
